All references to percentages paid will be subject to Usual and Customary, as defined by the policy. Up to $200 per vision exam and up to $75 per hearing exam for every two policy years. All covered vision services are paid up to $200 per person every two policy years. All covered hearing services are paid up to $500 per person every two policy years.
- Annual Deductible: $100 per policy year
- Deductible is waived for diagnostic and preventative dental, eye and hearing exams
- The single deductible covers all three benefit types: dental, vision, and hearing
- No waiting period for preventive exams or basic dental services
- 9-month waiting period applies to major dental services and all hardware (vision frames/lenses/contacts, hearing aids)
